The attached document is a guideline for use of antimicrobials in one hospital. The document title is "principles for use" as opposed to "guidelines" as a means of inspiring collaborative support. For the purposes of sharing, the topic title was selected as it best describes the document contents. This is one example and can be easily adapted for use in other facilities.

This evaluation has been made available to Kentucky hospitals as part of the MRSA elimination collaborative. There are two simple measurements that can be used in most facilities as an early indicator of antimicrobial use.
