Here is the checklist that we use each time a central line is inserted.
The listing of exclusion criteria for a subclavian site was developed in collaboration with our interventional radiologist. This checklist is included in our Grab It and Go CVL insertion packs. The nurse completes the tool at the bedside then places the tool on the chart where it remains until the chart is broken down by Medical Records. Medical Records places the tool in a box specified for Infection Prevention.
